Phyllis Louise Andreasen, age 82, died Sunday, June 5, 2022 at her home in Springdale. She was born July 21, 1939 in Ozark, Arkansas, to Jacob "Arlius" and Louise (Rich) Smith.  After Phyllis retired, she enjoyed visiting on the porch with friends and loved time with her family.

She was preceded in death by both parents, a sister, Wanda Faye Smith, and in-laws, James Roland Ford, Virginia Smith and J. Henderson Gossage.

She is survived by her two sons, Rick Culver and wife Amy, Scott Culver and wife Lisa, all of Springdale; brother Kenneth Smith of Ozark; sister, Ida Bea Gossage also of Ozark; grandchildren, Cody Culver, Shelby Henn, Mallory Bockelman, Philip Culver, Holly Culver; and two great-grandchildren, Ivy Bockelman, and Houston Henn.

Graveside services will be Thursday, June 9, 2022 at 2PM at Friendship Cemetery in Springdale.
